Mitchell C. Kulawik, D.D.S., 67, of Sioux City, passed away unexpectedly Tuesday, July 10, 2018, at his home.
Services will be 10 a.m. Saturday at Cathedral of the Epiphany Catholic Church. Burial will be in Calvary Cemetery. Visitation will begin at 6:30 p.m. today, with a vigil service at 7:30 p.m., all at the church. Arrangements are under the direction of Meyer Brothers Colonial Chapel. Mitch was born on Dec. 24, 1950, in Sioux City, the son of Mitchell R. and Marjorie (Kenner) Kulawik. Mitch graduated from Heelan High School in 1969, from Creighton University in 1973, and from Creighton University Dental School in 1977.
On June 2, 1973, Mitch married Mary Sylvester in Sioux City. After graduating from dental school, Dr. Kulawik and Mary returned to Sioux City and he began his dental practice in 1977 and was still practicing at the time of his death.
Dr. Kulawik was a member of Cathedral of the Epiphany Catholic Church. He had been a Boy Scout leader for 16 years and enjoyed vacationing at Lutsen, Minn.
Survivors include his wife, Mary of Sioux City; his children, Mitchell Wade Kulawik (Veronica) of Sioux City, Kasia Netley (Nick) of Lake Minnetonka, Minn., Craig Kulawik (Catie) of Omaha, Shannon Kulawik of Minneapolis, and Christopher Kulawik of Sioux City; and eight grandchildren, Sophie, Ambrose, Brandon, Amara, Jacob, Easton, Caiden and Finley.
Dr. Kulawik was preceded in death by his parents; and a grandson, Jackson.
